There are so many possibilities in a case like this. Fetal drug exposure. Reactive Attachment Disorder (RAD) is also another one that comes immediately comes to mind, given the family history. Of course, this is my experience-based, non-expert input.

Is he being referred for evaluation? Personally, I like evaluation and "labels," because they give us clearly-identified areas of need and help with our own expectations/frustrations. Also agreed that every child needs to be treated as an individual.
